Transtheoretical Model

This diagram is a state-transition diagram in a loop.
The first state is labeled Pre-Contemplation.
The next state is labeled Contemplation.
The next state is named Preparation.
The next state is named Action.
Action branches in two directions. One branch connects to a state named Maintenance.
The other branch connects to a state named Relapse.
Relapse and Maintenance both connect back to Pre-Contemplation.
Relapse also branches to a final state named Death.
Death is the only state that does not connect to any other states.
